Address : Enter address of Complainant

Mobile Number or Phone Number: Enter Mobile No of Complainant

E-Mail : Enter E-mail Address of Complainant

Subject: Select Subject from the Drop Down Menu. If your subject is not available in the list please choose “others”

Date of Occurrence: Select Date of crime occurrence

Place of Occurrence: Enter place of crime occurrence (Maximum 200 characters allowed in this field)

Complaint Description: Enter Complaint Description briefly (Maximum 2000 characters allowed)

Wants to attach documents: Chose Yes if you want to attach supporting documents. Chose No if you don’t want to attach documents. (Maximum file size should be 4 MB. File formats allowed are PDF, JPEG and PNG.

Security Code: Enter Security code displayed in the box

STEP 4: Finally, you get acknowledgment of your complaint.
